Runner's Knee (Chondromylasia)


 

How it feels

Increased pressure and pain on the underside of the knee cap.    This pain is usually more intense when the knee is bent while the muscles are tense.  This may develop into an arthritic problem where it feels like the knee cap glides over a rough surface.

The knee cap (patella) is attached with ligaments to the tibia and the quadriceps muscle and normally tracks in a groove over the femur.  If the knee is consistently exposed to rotational forces, the knee cap may be pulled off its centered track which leads to pain.  If this problem is not corrected degenerative arthritis may cause permanent problems and pain. Initially this pain my be primarily ligament pain, but prolonged runner's knee may cause cartilage problems.

How to eliminate the pain

Temporary relief can be found by placing a tension band around the leg right below the knee cap.  This helps the patella track better to reduce pain, but it only covers up the runner's knee symptoms.  In most cases protecting the knee from rotational forces works better and is a more permanent solution.  Hyperpronation causes increased rotational forces across the knee.

Hyperpronation

If you hyperpronate, your arches drop from non-weight bearing (sitting with your feet on the ground) to weight bearing (standing).  When your arches drop, your ankles roll in and your heels lean in (evert) as illustrated in the picture.  Even a small amount of hyperpronation can cause runner's knee.
