If you discovered that the tires on your car looked like this, you would be concerned because you instinctively know something is wrong. Wouldn't you naturally become concerned about your safety as a series of questions pop into your mind? Uneven shoe wear is cause for the same concerns. Although it doesn't carry the same risk, it can in fact be just as painful and even more costly. If you have proper foot mechanics, your shoes should wear evenly and slowly. If your shoes wear unevenly; if you grind down the sides of your heels; if you wear a hole underneath the second metatarsal (the bone connected to your second toe), or if you wear hardest along the outside edge of your shoes, your posture, foot and body mechanics are in need of repair. Just as with your car - if you ignore it, the price will just grow steeper as time compounds the problem. Your shoe wear patterns accurately reveal if you hyperpronate or supinate or do both. It will clearly show if you have a splayed (turned out) or pigeon toed (turned in) gait, or if you favor one leg over the other. Morton's Foot is quick to recognize by its deeper first web-space and/or a longer appearing second toe. If this describes your feet, you have Morton's Foot Syndrome, sometimes called Morton's Toe, which means you are predisposed to musculoskeletal dysfunction and pain. Dr. Dudley J. Morton, DPM, identified Morton's foot syndrome over 70 years ago. In about 10% of the population he observed that the first metatarsal (the foot bone that connects to the big toe) was frequently a little too short relative to the second metatarsal bone. He could clearly see that this foot problem changed the gait in a way that could cause pain and dysfunction elsewhere in the body. He even invented a special insole (Morton's extension) to help improve the gait, but because he missed a very important aspect of this foot structure, his invention was not effective. The real problem, however, is that the first metatarsal is slightly elevated and slightly rotated so it cannot support your foot properly. As it turns out everyone who has Morton's Foot also has an elevated first metatarsal. If you have an elevated first metatarsal, it means that your first metatarsal and big toe are not doing their job. Your big toe is supposed to carry most of your weight as you move over the forefoot on your way to a strong push off. But, your second metatarsal is doing the work instead. Since your first metatarsals do not properly support your weight, your feet become unstable. The foundation for your entire body becomes as wobbly as being on ice skates (Analogy by Dr. Janet Travell). So, now you can't stand, walk and run without extra effort. Every step takes a toll on your body and posture - not necessarily so much that you feel it now, but the accumulation of millions and millions of improperly aligned steps wears your joints and tires your muscles and eventually shows up as dysfunction and pain. Even if you consciously try to maintain good posture, your feet are not structurally capable of supporting your effort. Your body’s compensating response may be hyperpronation or supination (ankles rolling in or out).
You don't have to have a Morton's Foot to be at risk of
poor posture and its implications for pain. But, if you do have Morton's

Calluses are not always a silent warning because they can become quite painful. They are not a natural consequence of being on your feet all day. They are not a natural consequence of wearing shoes all day. You would more likely develop calluses if you spent a lot of time walking barefoot because calluses are the body's natural way of protecting your feet against abrasion. Shoes should protect your feet from abrasion so you should not have calluses. Incorrect foot mechanics can cause excessive pressure, rubbing and twisting inside your shoes that will result in calluses. They are caused by consistent and excessive friction between your feet and your shoe insole and shoe upper. Calluses indicate that your feet and your shoes do not get along. Shoes most often get the blame, but the fact is that your foot mechanics may be the major problem. If you take care of your foot problem, you'll get along better with all your shoes. Rummage through candid photos of yourself until you find a couple of full height profile pictures. Do you have a nice erect posture or do you look hunched forward? If you drew a straight line from right in front of the bump on your ankle to the center of your ear, does it bisect your knee, hips and shoulders? If not, your posture is off. Find a couple of frontal pictures of you from below the hips to your head. If you drew horizontal lines across your hips and your shoulders are they level and parallel? Is your head straight or leaning to one side? If you don’t have any pictures, ask someone to snap a couple of full height pictures of you from the front, back and side. Before the pictures are taken, do a couple of knee bends and stand as relaxed as you can while looking straight forward. Your posture is your window to your overall health.
